Privacy Statement
Processing of sensitive data is needed to provide a clinical psychology service and also to ensure the safety of patients if needs be. Patient files are only seen by Dr. Laura Haigh and not shared with other professionals.
Any letters or emails are written, or calls made, to other relevant professionals (e.g. GPs) will be with patient consent in order to keep patients safe if necessary, or update other professionals who are involved in your care. Please note that these could then become part of statutory files (e.g. NHS medical file) and that other professionals can then access these and provide you with suitable treatment. This will only ever be done in collaboration with the patient and with their consent unless they are at risk, someone else is at risk or a child is at risk.
You may ask any information held to be amended if inaccurate. This may need to be agreed between Dr. Laura Haigh and yourself and, if information has been shared, this information is to be corrected with whoever it has been shared with.
You will be asked give consent and permission to share my information but understand that you can withdraw consent at any time. However, please be aware that if you are at risk, someone else is at risk or a child is at risk, confidentiality can be broken and the only information that may be shared is that information which is required in order to keep you or someone else safe.
Patients have the right to ask to see and obtain a copy of their file and have a right to ask to restrict processing and to ask to erase information.
Raw information and/or process notes written in session by Dr. Laura Haigh are stored confidentially and are mainly to help with the process of therapy and these are my file notes. Raw information and/or process notes and letters are kept confidentially and securely under lock and key (or under two factor authentication password protection for electronic data) and may be destroyed after a 5 year period after treatment has ended. You have the right to read your notes if you request to do so.
